This is bad at so many levels that it boggles the mind. It could not have happened if even one person involved had actually read the letter before it went out.

I absolutely agree that somebody must be held accountable. This is exactly the same kind of benign negligence that occured at Walter Reed, and it implys that the Army learned nothing from that experience.
